Health History Interview Assignment
The health history interview assignment is an opportunity for you to apply
your knowledge of general pathology to a simulated clinical experience. From
your acquaintances, you will select a “client” with a common medical
condition covered in the PATH 268 course. You will then perform a health history
interview, analyze the data to determine the degree of control of the medical
condition, classify the client using the ASA classification system, and develop
a dental hygiene care plan. You will need to submit a video tape of the interview
and a written report with your analysis, classification, and care plan.

Instructions
- Select a “client.”
Select someone (friend, relative, neighbor) in your community who has a medical
condition. It is best to choose a person with a condition covered in the PATH
268 course and with whom you have a good rapport. Do not pick someone with
a complex health history that includes numerous diseases. Discuss the assignment
with the client and obtain consent for participation. Fax the consent form
to 306-798-9993 Attention: Dr. Scarfe.
- Obtain approval.
Submit your name and the medical condition of the client
you have selected to me for approval as soon as possible. Fax a copy of the
agreement of your client to participate in the assignment. I will inform
you if the client is acceptable.
- Prepare for the interview.
Generate a list of health history questions you
intend to ask. Consider using the generic health history questions from Lo01
Ls01. Be sure to include questions specific to the medical condition as well.
Have a peer provide feedback on your proposed questions. Revise your questions
if indicated.
Review the instruction sheet in Lo01 Ls01 on Interviewing clients.
Arrange
video taping equipment. If you do not have access to video equipment, contact
me and I will help you arrange to borrow equipment from SIAST.
- Conduct the interview.
Interview the client. Be sure to video tape the interview.
Make notes of the client’s responses.
- Write a report.
Content
Provide a comprehensive list of questions used during the health history
interview.
Analyze the data generated during the interview to determine if
the client’s
medical condition is in good control. Consider using the indicators of control
tool from Lo01 Ls02 as an aid to your analysis. Provide supporting evidence
of your assessment.
Use the American Society of Anesthetists Classification
system to classify the client. Give evidence to support your answer.
Determine
if you will proceed with dental hygiene care. If so, develop a dental hygiene
care plan.
Structure
- use point form and be concise
- use Times New Roman 12 point font and single space line spacing
- the report should be 2-3 pages in length
- Submit the video tape and analysis report.
