This Concept Map, created with IHMC CmapTools, has information related to: 10-29 CMAP Caddle, Decreased cardiac output related to decreased stroke volume as a result of blood flow obstruction by ventricular hypertrophy Intervention Explain low cholesterol/low fat dietary plan, Verapamil Classification Calcium channel blocker, antihypertensive, antianginal Diphenylalkylamine, Vital Signs 16:00 Temp:36.9 BP:112/69 HR:69 Resp:18 SpO2:98% RA, GI/GU 16:00 Abdomen is soft, no distention. Bowel sounds normoactive x4. No N/V. Continent. Voids independently. Characteristics of urine unknown., Gen. Chem. & LFT's Interpretation Levels are within normal ranges, Respiratory 16:00 Regular rate and rhythm. No adventitious sounds in any lobes. Oxygen saturation of 98% on room air., Decreased cardiac output related to decreased stroke volume as a result of blood flow obstruction by ventricular hypertrophy Intervention Encourage client to alternate between periods of rest and activity, Knowledge defecit related to cardiac catheterization procedure. Intervention Educate client and family on procedure and risks associated, especially risk of bleeding from groin site and the signs and symptoms that require medical attention., Cardiovascular 16:00 Heart rate 69BPM. No JVD. No carotid bruit. S1 and S2 sounds heard. Murmur heard. Radial and dorsalis pedis pulses present, regular, +2. Capillary refill ɛsec. No edema. Currently in NSR, Verapamil Nursing Indications Assess cardiac status and renal studies over long-term treatment. Increase fluid and fiber to prevent constipation. Limit caffeine consumption, no alcohol intake., CBC 10/27 WBC=6.2 RBC=5.39 Hgb=14.9 Hct=44.4 Plt=193, Activity intolerance realted to imbalance between oxygen supply and demand as evidenced by patient's complaints of shortness of breath. Intervention Monitor physiologic responses to increased activity level, HR, rhythm, and BP., Verapamil Brand Name Calan, Isoptin, Neuromuscular 16:00 A&Ox3, No muscle weakness, stable gait. Full ROMx4. PERRL, Integumentary 16:00 20 gauge normal saline lock. Appropriately moist and warm to touch, mobile with adequate turgor. No signs of breakdown on pressure areas. R groin site is soft, tender, no s/s bleeding or hematoma, Verapamil Reason for Recieving Symptom management of angina related to client's HOCM, CBC Interpretation Levels are within normal ranages, Verapamil Physician's Order 120mg PO BID, Activity intolerance realted to imbalance between oxygen supply and demand as evidenced by patient's complaints of shortness of breath. Intervention Develop a discharge plan with client & family to enable the client to continue functioning at maximum level of activity tolerance., Left/Right Heart Cath and Echocardiograph 10/29 Impressions: Hypertrophic Cardiomyopathy, severe. Follow-up in 1-2 weeks. 60ml Ultravist used. Echocardiograph on 10/14 showed a LVEF 60%, dynamic outflow obstruction.
